POSITION SUMMARY

We are seeking a dynamic and organized Case Manager to provide vital direct support, advocacy, and service coordination for youth and families in our residential programs. In this role, you will act as a central pillar of our care team. You will be partnering closely with Clinicians and the Clinical Director to ensure seamless daily operations, rigorous compliance, and exceptional resident care from intake through transition.

KEY RESPONSIBILITIES

1. Intake, Admissions & Transition Planning

  • Streamline Admissions: Fast-track referral reviews within 24 hours, participate in candidate interviews, and coordinate all acceptance logistics (including transportation).
  • Rapid Intake Processing: Complete comprehensive intake paperwork, safety plans, and medical/medication documentation within 8 hours of arrival.
  • Drive Discharge & Aftercare: Outline and oversee transition and discharge planning in close alignment with DCF, clinicians, and external providers.

2. Clinical Documentation & Compliance

  • Manage Compliance Timelines: Draft, update, and secure signatures for critical state-regulated assessments (including YRAT, CANS, and Biopsychosocials) within strict 72-hour, 20-day, and 30-day deadlines.
  • Database Management: Utilize the Virtual Gateway system and MyEvolv to track treatment plans, incident reports, and resident budgets in accordance with EEC, DCF, and Caring Together standards.
  • Audit Readiness: Maintain pristine, organized, and signed client binders and communication logs.

3. Client Advocacy & Daily Operations

  • Coordinate Care Networks: Serve as the primary liaison for DCF, schools, and healthcare providers; facilitate weekly progress summaries and lead collaborative team meetings.
  • Life Skills & Group Facilitation: Schedule and lead resident groups focused on budgeting, independent living (PAYA), and parenting skills.
  • Daily Living Support: Monitor residents' quality of life (hygiene, nutrition, apparel) and actively participate in house operations, including meal preparation and maintaining a clean, welcoming environment.

4. Leadership & Program Administration

  • Staff Mentorship: Partner with the Program Director to audit staff documentation, facilitate in-house trainings, and model positive leadership for both staff and residents.
  • Personnel Support: Assist in interviewing and onboarding new job candidates and interns.
  • On-Call & Mobility: Participate in the Manager On-Call rotation (logging activity via Asana) and utilize your personal vehicle to transport youth under 18 to vital court, medical, or academic appointments.

JOB REQUIREMENTS

  • Dynamic Engagement: Proven ability to work effectively with youth and families in fast-paced residential, transitional, and outreach settings.
  • Regulatory Knowledge: Familiarity with state systems (DCF, EEC) and standard assessment tools (CANS, YRAT) preferred.
  • Reliable Transportation: Valid driver’s license and a reliable personal vehicle for resident transport and community outreach.

  

Qualifications:

QUALIFICATIONS & REQUIREMENTS

Education & Experience

  • Degree: Bachelor’s degree in Psychology, Social Work, or a related field preferred.
  • Experience: Minimum of 2 years of experience working in a residential setting preferred.
  • Availability: Flexibility to work adaptable hours to meet program and resident needs.
  • Mobility: Valid driver’s license and access to a reliable personal vehicle.

Required Certifications & Training

Candidates must pass an initial competency evaluation and maintain ongoing professional development certifications, including:

  • CPR / First Aid
  • Dynamics of a Crisis & Physical Restraint Training
  • Medication Administration Program (MAP)
  • Trauma-Informed Care & Collaborative Problem Solving
  • HIPAA, OSHA, Ethics, and Boundaries

LEADERSHIP & MENTORSHIP

  • Staff Development: Provide weekly supervision to designated staff; utilize the "coach, model, observe" method to incrementally build their skills and master documentation requirements.
  • Professional Growth: Receive direct mentoring from the Clinical Director, Clinicians, and Program Director to learn broader program operations and prepare for advancement opportunities within the organization.
