About the IMT Program
In January 2016, the NYC Department of Health and Mental Hygiene (DOHMH) launched an innovative initiative to improve outcomes for individuals with significant involvement in homeless services, the criminal justice system, and behavioral health systems. As part of this initiative, Intensive Mobile Treatment (IMT) Teams were created to serve individuals with the highest and most complex needs.
Due to the program’s success, the IMT model has expanded over time and continues to grow. Each team maintains a small caseload of up to 27 participants, allowing for intensive, individualized support. Services are provided for as long as clinically indicated, with most engagements extending beyond one year.
Each IMT team is multidisciplinary and includes social service clinicians, peer specialists, a psychiatrist, and a full-time Registered Nurse, all working closely together to support client stability, recovery, and overall health.

Clinicians receive expert supervision and strong peer support. We offer a competitive salary commensurate with experience, excellent benefits, and a 5% salary differential for service on an IMT Team, which is already reflected in the posted salary range. The typical schedule is Monday through Friday, 9:00 AM–5:00 PM, with flexibility based on program needs.
Position Summary
The IMT Registered Nurse plays a key role in supporting the overall health and wellness of program participants by integrating physical and behavioral health care. Working alongside the multidisciplinary team, the RN provides nursing care across community and institutional settings and supports continuity of care for individuals with complex needs. The role includes participation in an on-call rotation consistent with the IMT model and program needs.
Key Responsibilities
- Provide nursing assessments and clinical care in a variety of settings, including participants’ homes and institutional environments
- Administer medications and deliver nursing interventions under the direction of the IMT psychiatrist
- Maintain accurate medication records, logs, and clinical documentation
- Support participants with treatment adherence, including medication management
- Coordinate care and link participants to primary and specialty medical services
- Participate in multidisciplinary team meetings and care planning
- Communicate and collaborate with pharmacies, hospitals, and city agencies as needed
