The Center for Urban Community Services, (CUCS), is an acclaimed human services organization and one of the nation's largest providers of housing and social services. CUCS is a leader in implementing new practices, sharing knowledge and assisting in shaping local, state and national strategies, so that persons who are homeless, low-income, living with mental illness, or have other special needs can live successfully in the community.
CUCS invites applications for a Medication Case Manager position for The Park House and Webster residence. CUCS provides services for 20 units at Park house and 90 units at The Webster formerly homeless and special needs individuals, including those living with HIV/AIDS. The site is located in the Tremont neighborhood in the Brox and consists of two buildings located next to each other.
CUCS provides comprehensive on-site social services using a recovery orientation, which includes evidence-based practices with particular emphasis on mental health and substance use recovery.
Medication Case Manager
The Medication Case Manager provides person-centered support to clients with medication management, adherence, and treatment engagement. Services are delivered both onsite and in the community.
Key Responsibilities, include but are not limited to:
- Support clients with safe self-administration of medications and promote medication literacy to increase independence in managing their treatment
- Coordinate with pharmacies, medical providers, and internal teams to ensure timely ordering, delivery, and receipt of client medications
- Monitor client response to medications and collaborate with the clinical team to adjust support plans as needed
- Accompany clients to medical, psychiatric, and pharmacy appointments as needed to support treatment adherence
- Facilitate psychoeducation and medication management groups in alignment with agency standards
- Maintain medication room organization and ensure compliance with all applicable policies and regulations
- Complete accurate and timely documentation across agency databases and funder systems, including progress notes and required service records
- Participate in QA and internal audits in coordination with onsite management
- Escalate concerns promptly to supervisory staff in accordance with program protocols
