USA The Center for Human Development, (CHD) is seeking a Team Supervisor
for our Adult Community Clinical Services (ACCS), located in
Springfield, MA. ACCS is funded through the Department of Mental Health
and is a multidisciplinary outreach approach for persons served in need
of psychiatric and other services living independently in the community.
This involves active engagement including assertive outreach, consistent
assessment and treatment planning, skill building, symptom management,
addiction treatment, and behavioral and physical health monitoring and
support. YOUR ROLE AS A TEAM SUPERVISOR: ACCS teams include licensed
clinicians, RNs, housing specialists, substance use specialists, direct
care staff, and peer/family specialists. The Team works collaboratively
to provide person-centered, evidence-based services in support of
participant\'s treatment goals. The Team Supervisor assists participants
in need of psychiatric services acquire the skills necessary to live
independently in the community. S/he works within the Agency\'s team
model, establishing open communication, support, and accountability
among co-workers. REQUIRMENTS: Masters prepared unlicensed and/or very
experienced Direct Care working with the adults experiencing mental
health symptoms (min. 2-3 years\' experience) Supervisory experience
preferred ESSENTIAL JOB FUNCTIONS: Provides direct support and
supervision to Direct Care I and II staff Provides crisis intervention
Adheres to program policy and procedure manual Participates in team
approach, sharing responsibility for participants services Notifies
senior supervisory staff immediately in the event of a crisis, suspected
illegal activity, or any behavior constituting an incident in the
program Assists participants with activities of daily living e.g.
personal hygiene, meal preparation, housekeeping, laundry, money
management, etc. Transports participants in matters related to service
delivery, e.g. medical appointments, shopping, job interviews,
vocational program, banking, laundry Administer medication accordance
with MAP policies and procedures Deals with entry and termination issues
