Description The Center for Human Development, (CHD) is seeking Outreach
Workers for our Adult Community Clinical Services (ACCS), located in
Springfield, MA. ACCS is funded through the Department of Mental Health
and is a multidisciplinary outreach approach for persons served in need
of psychiatric and other services living independently in the community.
This involves active engagement including assertive outreach, consistent
assessment and treatment planning, skill building, symptom management,
addiction treatment, and behavioral and physical health monitoring and
support. A generous sign on bonus of \$1,500 makes this an opportunity
you do not want to pass up! YOUR ROLE AS DIRECT CARE: ACCS teams include
licensed clinicians, RNs, housing specialists, substance use
specialists, direct care staff, and peer/family specialists. The Team
works collaboratively to provide person-centered, evidence-based
services in support of Persons? treatment goals. The Direct Care Staff
assists participants in need of psychiatric services acquire the skills
necessary to live independently in the community. S/he works within the
Agency?s team model, establishing open communication, support, and
accountability among co-workers. REQUIRMENTS: At least one year
experience working with population preferred Bachelor?s degree from an
Accredited Educational Institution Preferred (required For DC II, Or
additional experience) High School diploma required Preferred candidates
are Bilingual (English/Spanish), with ability to read/write fluently.
ESSENTIAL JOB FUNCTIONS: Assists participants with activities of daily
living e.g. personal hygiene, meal preparation, housekeeping, laundry,
money management, etc. Transports participants in matters related to
service delivery, e.g. medical appointments, shopping, job interviews,
vocational program, banking, laundry Administer medication accordance
with MAP policies and procedures Maintains records in accordance with
standard format and timetables to include assessment and progress
reports/service notes, incident reports, money disbursements, and other
specialized documentation as required Provides for and participates in
recreational activities, e.g. dining out, spectator sports, movies, etc.
for the participants Participates in the development of treatment plans
