The Center for Human Development is seeking a dedicated Case Manager to provide outreach and support services to clients with psychiatric or substance use disorder diagnoses. As a vital member of our team, you will play a crucial role in enabling clients to access clinical treatment services and other necessary supports.
Key Responsibilities:
- Coordinate services and linkages to the community
- Provide mobile outreach services in various settings, including inpatient units, homes, day programs, etc.
- Assist clients in obtaining benefits, housing, and healthcare as needed
- Offer crisis support and develop crisis prevention and/or safety plans
- Encourage the utilization of clients' natural support systems, recovery-oriented peer support, and self-help supports and services
- Contribute to data collection and tracking for grant evaluation
Requirements:
- Bachelor's degree from an accredited educational institution or three years of relevant work experience
- CPR and First Aid certification
- Valid driver's license and reliable transportation for business use
- Ability to counsel and support clients with psychiatric and/or substance use disorder diagnoses
- Demonstrated knowledge of trauma-informed care
- Lived experience relative to psychiatric and/or substance use disorder (desired)
