Benefits

• Medical Insurance-Two health insurance plans (HSA or PPO co-pay) are available to choose from for staff with weekly scheduled hours of 32 or more as well as power weekend employees. Coverage is effective the first day of the month following employment.

• Employee Assistance Program (EAP) CHC provides an EAP to help non-PRN employees, their spouses and eligible children in coping with personal problems and stress. CHC will pay the cost of four counseling sessions per employee and eligible family members each calendar year. This is effective immediately upon employment. Employees may contact the EAP provider to directly schedule confidential appointments. The number of employees participating is the only information shared with CHC.
• Healthy Lifestyle Benefit- Employees who participate in CHC’s medical insurance are eligible for reimbursement of 50% of the cost up to $300 per calendar year for participating in the following: gym membership (single membership rates), weight loss counseling or education, organized exercise programs such as Zumba, Yoga or Pilates.
• 403B / Roth 403B- Employees regularly scheduled to work 20 hours or more per week are eligible to participate in CHC’s retirement plan following completion of their 90-day probationary period. Vesting is 100% upon enrollment. CHC will match at 25% the first $16,000 of the employee’s contribution annually.
• Accidental Death and Dismemberment Insurance- CHC provides one times the annual earning in AD&D insurance for staff with weekly scheduled hours of 32 or more as well as power weekend employees. This is effective the first day of employment.
• Life Insurance- Effective the first day of employment, CHC automatically provides life insurance equal to one time the employee’s annual salary or a minimum of $20,000 for power weekend employees and all staff with weekly scheduled hours of 32 or more.
• Employee Peer Recognition program
• Annual anniversary/retention bonus - Upon completion of each year of employment, CHC will give $100 per year with a maximum of $1,000
• Competitive Time Off- CHC’s paid time off (PTO) policy provides employees with flexible earned time off for vacation, illness, personal matters and other reasons. Staff with weekly scheduled hours of 32 hours or more are eligible to accrue PTO.

Patient Care for CNA Home Hospice Care:

• Travel to where Patient lives and calls home.
• Provides care and services to patients according to the care plan or as approved by an RN.
• Assists with other activities of daily living as directed by an RN.
• Offers support and encouragement to patients and families.
• Functions within acceptable time frames for daily patient visits.
• Clearly communicates information regarding changes in the patient’s mental/physical condition, complaints, or special requests to the Agency nurse, and ECF unit manager if applicable.
• Demonstrates good judgment in knowing what information needs to be communicated.
• Handles communication in a timely manner.
• Maintains documentation of patient care provided on both Agency forms, and the ECF record if applicable.
• Uses appropriate Mobile Care documentation module or paper forms.
• Records information accurately, thoroughly, and in a timely manner
• Committed to Agency resolution against fraud and abuse.
• Knowledgeable in hospice philosophy and state and federal rules and regulations.
• Respects confidentiality of patients/families and other Agency employees.
• Attends educational meetings/conferences for professional growth as required.
• Follows directions and policies that allow for an organizational commitment to working under time pressures without sacrificing quality.

Successful Capabilities of CNA Home Hospice Care:

• Must be able to read and write English proficiently.
• Must be state tested nursing assistant in good standing
• Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.

Conditions of Employment for CNA Home Hospice Care:

• Prior training and experience as a Home Health Aide with Indiana certification as a Nurse Aide.
• Must pass Drug Screen and Background check.
• Compliance to Annual Flu Shot and Covid Vaccination Policy or ability to provide exemption documentation
• Pass TB test
• Provide an active auto insurance policy as a licensed driver in the State of Indiana with the proper level of coverage as directed
• Provide an active driver’s license
• Ability to provide proof of eligibility to work in the United States.
• High level of integrity, ethics and professionalism.
