What You’ll Do:
- Provide skilled nursing visits to hospice patients in their homes
- Conduct clinical assessments and address symptom management needs
- Collaborate with the interdisciplinary team to support patient care plans
- Educate and support patients and family caregivers
- Maintain timely and accurate clinical documentation
Schedule:
Flexible per-diem role
Work 8–24 hours per week based on availability and organizational needs
What We’re Looking For:
- Current New Jersey RN license
- Strong clinical judgment and communication skills
- Ability to work independently in a field setting
- Compassion for hospice and palliative care patients and families
- Prior hospice, palliative, or home care experience preferred (not required)
