The Center for Hope Hospice and Palliative care is a unique Union County based non-profit organization seeking a per-diem Advanced Practice Nurse (APN) to conduct in-hospital evaluations of patients.
This role is ideal for a compassionate and experienced APN who is interested in hospital-based consults with a focus on assessing patients’ medical status, symptom burden, as well as readiness and appropriateness for palliative care services. You will work directly with hospital staff, patients and families, and the Center's Palliative Care Certified Medical Director to determine clinical appropriateness for palliative care, while helping set expectations and support long-term care planning.
Key Responsibilities:
- Conduct bedside evaluations of hospitalized patients to assess for palliative care management recommendations.
- Collaborate with hospital physicians, discharge planners, and our Medical Director
- Offer symptom management recommendations and communicate prognosis and care options
- Document assessments and recommendations in the EMR
- Support families and patients through difficult conversations about advanced care planning and completion of POLSTs, with empathy and clarity
Minimum Qualifications:
- Master’s degree in Nursing.
- Current Registered Nurse license and Nurse Practitioner license.
- Active CDS and DEA Licenses
- Current CPR Certification
- Minimum of 3 years NP experience
- Palliative care experience preferred
Location:
This position will require the ability to reliably commute to the Center's offices in Scotch Plains as well as to make in-person visits to Robert Wood Johnson in Rahway, NJ and Trinitas in Elizabeth, NJ.
Compensation
This is a per-diem role, which will be compensated on a fee-for-service basis, as well as an hourly rate for all non-billable hours.
Hospital Evaluations - $75 per visit
Face to Face Recertifications - $100 per visit
Non-Billable Meetings - $40 per hour.
