Salary: $25.82 per hour

POSITION SUMMARY:

Under the direction of the Program Director and Family Care Counselor, the Family Resource Specialist is responsible for direct in-home case management services on behalf of the family. This job requires a high level of sensitivity and communication skills.

Duties and Responsibilities:

• Provides direct services to a caseload of 12-13 families utilizing KFT program model.
• Provides in-home case management services to meet the family's goals on the CARE plan. These goals may include but not limited to; parenting skills, household management, communication skills, conflict resolution, child development, basic budgeting, household management skills, and structuring daily routines.
• If a family is in the process of reunification, Specialist will act as a support for the parents and assist with any transitional issues that occur to assist in maintaining reunification.
• Provide linkage to community resources which can include but not limited to; child care, educational needs or services, food stamps, cash assistance, WIC, Supplemental Security Income, Social Security Disability, health care, treatment, and Division of Developmental Disabilities
• Connects families and youth to Family Success Centers, sports and recreational activities, creative arts and educational programs, and assists in providing special events for the families.
• Transports individuals/families to appointments, meetings and activities which are part of their Family CARE Plan.
• Demonstrates for families how to advocate for themselves and their children within the community to help establish self-sufficiency.
• Upon assignment of families, contacts families within specific time frames given by the Program Director and/or Family Care Counselor.
• Provides a support system to families in recovery seeking to improve their overall mental and physical health
• Plans efficient transportation routes for individuals to any appropriate services.
• Confers with Family Care Counselor on any significant information that may require immediate attention.
• Exhibits knowledge of client's psychosocial and medical needs as appropriate.
• All required programmatic paperwork must be completed thoroughly and accurately within specific timeframes as designated by program design.

• Meets regularly with supervisor for individual and group supervision for case review, staff development, and program development and participates in other continuing professional development opportunities.

REQUIREMENTS:

• Bachelors in Social Work or equivalent degree
• At least two years of experience working with program population
