POSITION SUMMARY:

The Care Coordinator is a key member of the Crisis Response Team, responsible for supporting individuals and families experiencing behavioral health crises. The Care Coordinator ensures timely linkage to services, coordinates follow-up care, facilitates communication among providers, supervises Peer Specialists, and monitors client progress post-crisis to support stabilization and recovery. This role requires strong crisis management skills, excellent interpersonal communication, and the ability to navigate complex behavioral health systems in high-stress environments.

Duties and Responsibilities:

Initial Engagement and Assessment

  • Collaborate with clinicians and Peer Specialists during or immediately following a crisis event.
  • Gather client demographic and psychosocial information.
  • Support safety planning in collaboration with clinicians and the client.
  • Complete required documentation within established timeframes. 

Service Coordination and Linkage

  • Facilitate referrals and warm handoffs to behavioral health providers, medical professionals, housing services, or other community supports.
  • Schedule and confirm appointments; assist clients with transportation or logistical barriers.
  • Maintain relationships with system partners and regularly update shared databases of services.
  • Maintain an inventory of community resources and update the real-time database of available treatment services.

Follow-Up and Monitoring

  • Conduct follow-up calls or visits (within 24-72 hours) to ensure engagement in services.      
  • Track client progress and ensure continuity of care for up to 30 days post-crisis.
  • Re-engage clients who miss appointments or need additional support.
  • Coordinate with the Data Manager to support program evaluation and reporting.

Supervision and Peer Support Coordination

  • Supervise Peer Specialists and ensure effective collaboration with counselors. 
  • Partner with Peer Specialists to provide education to citizens and stakeholders about available recovery and social services.

Collaboration and Communication

  • Act as a liaison among clients, families, service providers, and the Crisis Response Team. 
  • Participate in team meetings, case consultations, and multidisciplinary planning. 
  • Communicate effectively with all stakeholders during and after a crisis.

Crisis System and Program Support

  • Support development and implementation of crisis response and contingency plans. 
  • Assist in conducting risk assessments and updating response plans. 
  • Coordinate care for high-utilizers and clients with co-occurring disorders or complex needs. 
  • Lead debriefing sessions post-crisis to identify areas for improvement. 
  • Support the implementation of emergency drills and simulations. 
  • Assist with coordination of logistics and resources during crises. 
  • Maintain knowledge of best practices in crisis intervention and trauma-informed care.

Qualifications: 

  • Bachelor's degree in Social Work, Psychology, Human Services, or related field (Master's preferred).

  • Minimum 2 years of supervisory experience.

  • Minimum 3 years of experience working with individuals with behavioral health challenges, substance use, or co-occurring disorders.

  • Experience with crisis services, EMS, emergency departments, or community-based response preferred.

  • Strong understanding of behavioral health systems, community resources, and crisis management best practices. 

  • Excellent communication, documentation, and organizational skills.

  • Proven leadership ability and experience working with diverse teams and stakeholders.

  • Ability to make sound decisions under pressure and work flexible hours, including nights and weekends.

  • Experience with trauma-informed care and recovery-oriented systems. 

  • Valid driver's license, insurance, and clean driving record; ability to operate agency vehicles.

  • Ability to lift up to 50 pounds.
