Your Day-to-Day
-
Provide individual, family, and group counseling, as well as crisis intervention
-
Complete and document mental health screenings and assessments
-
Develop and maintain treatment plans with patient participation
-
Deliver clinically appropriate behavioral health treatment and support

Assist primary care providers in identifying and addressing mental health and psychosocial concerns
-
Support patients with behavior change, self-management skills, and lifestyle goals
-
Identify barriers to treatment plan follow-through and help connect patients to additional services

Lead patient education and support groups

Collaborate with care team members to coordinate patient care

Maintain timely and accurate documentation

Participate in peer review, case consultation meetings, and quality improvement initiatives
What Makes You a Great Fit
-
Master’s degree in social work with limited or full licensure, or Licensed Professional Counselor licensure required

Minimum of 2 years of Social Work experience preferred

BLS certification required or ability to obtain

Strong understanding of behavioral medicine and evidence-based treatment approaches

Comfortable working closely with primary care providers in a team-based setting

Strong clinical assessment, communication, and documentation skills

Excellent organizational, customer service, and time management skills
-
Ability to maintain strict patient confidentiality
