What are the responsibilities and job description for the Case Manager position at CENTER FOR FAMILY DEVELOPMENT?
Center for Family Development (CFD), an outpatient behavioral health center, seeks a full-time Case Manager to work as a member of a multi-disciplinary team. The Case Manager will support clients in our mental health programs by coordinating community resources to address clients' needs. This includes supporting access to substance use treatment, medication management, physical health/medical services, mental health supports, benefits and entitlements, and supports for basic needs.

Job duties include:
- Provide case management, care coordination, and skill-building interventions aligned with individual treatment goals.
- Support individuals and families in accessing mental health, substance use, medical, housing, educational, and social service resources.
- Assist individuals with navigating the State's benefits enrollment and eligibility site (OregonOne), as needed.
- Coordinate care with behavioral health providers, primary care providers, schools, and community agencies.
- Assist individuals in organizing, attending, and following up on appointments.
- Advocate for individuals and families within service systems.
- Assist individuals in identifying and resolving barriers to care.
- Support development of self-management, coping, and daily living skills.
- Document services accurately and timely in the electronic medical record and required tracking systems.
- Track caseload activity, service outcomes, and required metrics.
The ideal candidate has the following attributes:
- Basic knowledge of Motivational Interviewing, Stages of Change, and Harm Reduction.
- Demonstrated knowledge of community and social service resources, including health services (substance use, mental health, and physical health), housing, employment, benefits, and other social services.
- Computer literate, with basic knowledge of Microsoft Office Suite, as well as a high level of initiative in keeping current with documentation within the electronic health record.
- Ability to be self-directed to prioritize workload and complete tasks in a timely and efficient manner.
- Ability to set boundaries, resolve interpersonal conflict and de-escalate issues.
- Dependability, willingness to learn, has a cooperative approach to problem-solving.
- Has experience working with individuals experiencing houselessness and other factors related to social determinants of health.
Eligibility Requirements:
- Credentialed, or ability to gain credential, as a Qualified Mental Health Associate (bachelor's degree in a behavioral sciences/mental health field, or a combination of at least three year's relevant work, education, training or experience).
- Successful completion of background check required upon hire.
