Position Summary:
Provides case management services to eligible consumers under the Pennsylvania DPW Code Title 55 Base Service Unit Service Description.
Essential Functions:
·Conduct comprehensive assessments of consumers to determine needs including Social Determinates of Health, Mental Health assessments and Housing Needs.
·Develop a comprehensive treatment plan for persons determined to be in need of service.
·Ensure continuity of the services for the consumer and coordinate the services received.
·Complete all reports, documentation and data entry per CCR policy.
·Maintain utilization requirements.
·Provide follow up to identified consumers
·Complete outreach as assigned.
·Other duties as assigned to fulfill the mission of the agency.
Daylight hours, Monday-Friday. Some flex schedule and or work from home hours. Travel required.
Mileage reimbursement is available at the state set rate! The starting rate
for this position is $21.54/hr Depending on experience and education.
Qualifications:
A bachelor's degree in human services or a related field; AND one-year mental health experience.
Valid driver’s license, proof of auto insurance, Act 33/34 and FBI clearances will be required. Individuals who have accessed human services are encouraged to apply. EOE/ADA
