Position Summary: Provides case management services to eligible consumers under the Pennsylvania DPW Code Title 55 Base Service Unit Service Description.

Essential Functions:
- Conduct comprehensive assessments of consumers to determine needs.
- Develop a comprehensive treatment program for persons determined to be in need of service.
- Ensure continuity of the services for the consumer and coordinate the services received.
- Complete all reports, documentation and data entry in a timely and accurate fashion.
- Maintain utilization requirements
- Provide follow up to identified consumers.
- Able to manage time effectively to meet the needs of consumers and program requirements.
- Be well organized, have excellent verbal and written communication skills.
- Computer competency with the ability to learn agency programs.
- Other duties as assigned to achieve the mission of the agency.
Daylight, Monday-Friday hours, Flex schedule, No Weekends. Mileage reimbursement is available at the state set rate!
The starting rate is $21.54/hour but may start higher depending on experience and education.
Qualifications:
- Bachelor’s degree in Human Services or related field.
- One-year mental health experience or related position.
- Valid driver’s license, proof of auto insurance, Act 33/34 and FBI clearances will be required.
