Position Summary:
Provides case management services to eligible consumers as defined by DPW Title 55 Code Chapter 5240.

Essential Functions*
- Provide blended case management services to individuals in the community who qualify for services based on a mental health diagnosis.
- Complete all reports and documentation in a timely fashion.
- Maintain quality standards.
- Participates in an On-Call Rotation to ensure 24-hour coverage for holidays, weekends and daily, between the hours of 4:30 pm-8:30 am.
- Other duties as assigned.

- Office, Community, Agency, and Remote setting with frequent computer use.
- Frequent travel in the community; hospitals, schools, collateral agencies, or family/consumer homes.
- BCMs (Blended Case Management) will be available to meet with consumers during normal business hours, as well as in the evenings and on weekends when scheduled, to meet the needs of the individual or when an individual is experiencing a crisis event. An on-call BCM (Blended Case Management) will also be available via the on-call hotline 24 hours a day, 7 days a week.

- Sufficient mobility to work in a normal office setting and use standard office equipment including a computer and navigation in, out and through various structures which may not be handicapped accessible.
- Vision to read printed materials and VDT screen.
- Hearing and speech sufficient to communicate in person or over the phone.
- Lifting to 20 lbs. maximum and occasionally carrying such articles as a laptop computer, files, ledgers, and small tools.
- Walking, standing and stair climbing will be necessary in carrying out duties.
The starting rate is $24.00-$26.00/hour depending on experience and education. We pay a stipend of $250.00 per week when covering on call for one full week.
Qualifications:
Bachelor’s degree in Human Services or related field. One year experience in MH Case Management or in the mental health field. Knowledge of case management principles, computer literate, familiarity with the Recovery Model, and knowledge of Schuylkill and Northumberland County resources.
Valid driver’s license, proof of auto insurance, Act 33/34 and FBI clearances will be required. Individuals who have accessed human services are encouraged to apply. EOE/ADA
