Job Title: Team Leader, Young Adult Assertive Community Treatment

Job Summary:                                                   

The Young Adult Assertive Community Treatment (YA ACT) team is a multi-disciplinary team that provides evidence-based, mobile, community-based mental health treatment to promote the recovery and community integration of individuals aged 18 to 25 with serious mental illnesses, high rates of co-occurring substance use disorders and high utilization of emergency/crisis services. The team also provides developmentally appropriate support to build and implement productive vocational or education plans and will focus on enhancing necessary skills for independent living and self-management. 

The Young Adult Assertive Community Treatment (YA ACT) Team Leader directs and supervises YA ACT staff activities, leads organizational, clinical and service planning meetings, and coordinates administrative functions. In collaboration with the team’s psychiatrist, the Team Leader provides clinical direction to staff. The leadership results in effective treatment and rehabilitation services to reduce episodes of harmful outcomes such as use of emergency rooms, medical and psychiatric inpatient beds, arrests and incarceration for program consumers. The Team Leader guides the team to deliver positive behavioral health, social and individual outcomes such as consumer participation in education, competitive employment, and family and social relationships. 

Shift Hours: This position is full-time Monday – Friday from 9:00 AM – 5:00 PM.

Salary: The salary range for this role is $95,000-$97,850.

Location Address: 424 East 147th Street, Bronx, NY 10455. Our office is easily accessible by public transportation.

Workplace Flexibility: Flexible - Staff have the flexibility to complete their workday from home or another suitable space, typically after reporting to the office and/or engaging in field or community-based work. One virtual team meeting per week.

What You Will Be Doing: 

  • Oversees the clinical and administrative operations of the YA ACT Team, including but not limited to daily team meetings, regular clinical meetings, timeliness and quality of clinical documentations and reporting, intakes and discharges, team’s after-hour emergency line, use of service dollars, and team members’ schedules. 
  • Ensures the team delivers culturally and developmentally appropriate, trauma-informed, recovery-oriented services and effectively applies motivational interviewing. 
  • Provides regular clinical supervision, training and professional development opportunities for team members. 
  • Provides direct clinical care and services to consumers in the community when needed. 
  • Ensures the team meets its performance outcomes and operational standards as defined by New York State Office of Mental Health (OMH) licensing standards and New York City Department of Health and Mental Hygiene (DOHMH) contracting standards. 
  • Oversees team’s incident reporting procedures according to OMH standards and ensure team adheres to the Incident Review Committee (IRC) policies and procedures. 
  • In collaboration with the Quality Assurance department, conducts utilization review and quality assurance reviews to ensure that the YA ACT team operates in compliance with New York State OMH guidelines and regulations, New York City DOHMH operating standards, as well as HIPAA confidentiality regulations. 
  • Consults with senior clinical leadership on complex, high-risk cases, as well as team and personnel issues. 
  • Coordinates monthly Medicaid billing using electronic medical record to select billable progress notes and ensure team achieves billing targets defined by program budget. 
  • Help ensure we maintain a safe working environment by reporting any hazards or unsafe conditions, e.g., liquid spill in hallway, to the appropriate facilities staff member or supervisor.
  • Performs other duties as required by Director and executive staff. 

What We Are Looking For: 

  • LMSW, LCSW, RN, LMHC, or Licensed Psychologist (PsyD/PhD) 
  • Two years of supervisory experience. 
  • At least three years of experience providing services to adults with serious mental illness. 
  • Experience providing services to young adults (ages 18 to 25). 
  • Experience working in community mental health settings that provides services to recipients in-vivo in the community. 
  • At least two years of experience working on a multidisciplinary team. 
  • Knowledge of developmental issues and needs for young adults (age 18-25), family systems, vocational and educational planning. 

Additional, preferred skills we are looking for but are not required: 

  • Fluency in Spanish.
