Lead With Purpose
Rio Grande Rehabilitation & Healthcare Center is seeking a licensed Physical Therapist (PT) or Occupational Therapist (OT) to serve as our Director of Rehabilitation. In this leadership role, you’ll oversee therapy operations, support our dedicated team, and help residents reach their highest potential in a warm, long-term care environment.

Your Responsibilities:
- Direct and manage daily rehab operations (PT, OT, assistants, aides)
- Ensure quality therapy services are aligned with patient goals and care plans
- Monitor documentation, compliance, and therapy productivity
- Collaborate with nursing, MDS, and care teams for coordinated care
- Support and mentor your team with hands-on leadership
What You’ll Need:
- Active Colorado PT or OT license (or compact state eligibility)
- 2 years of SNF or LTC therapy experience preferred
- 1 year in a supervisory or lead therapist role
- Strong communication, organization, and leadership skills
- Familiarity with PDPM and electronic documentation systems
