Program: Assertive Community Treatment (ACT)

Location: Community-based role serving Clearfield, Elk, and Jefferson counties, with services provided in the office, community, and clients' homes.

Employment Type: Full-time

Sign On Bonus: $1500 ($750 upon hire and $750 post 6 months employment in the position)

Compensation: $23.02 - $26.39 per hour

About the Role:

The ACT team provides comprehensive, locally based treatment to adults with serious and persistent mental illnesses.

Our ACT Team is seeking a Substance Abuse Specialist to join the team. In this role, you will support individuals served by the ACT program while working closely with team members to ensure consistent, high-quality care.

What You'll Do:

  • Work 40 hours per week, 52 weeks per year, may include non-traditional hours
  • Travel within the ACT service area (travel required; mileage/time reimbursed per policy)
  • Maintain the highest level of confidentiality and represent the agency with professionalism at all times.
  • Participate in required meetings, training sessions, and public-relations activities.
  • Perform additional duties as assigned to support program and agency operations.
  • Provide consistent, high-quality, recovery-oriented, and intensive, individualized and comprehensive treatment in line with agency philosophy and best practices.
  • Promote a physically and emotionally safe, welcoming therapeutic environment.
  • Work cooperatively with the ACT team to meet the individual needs of clients.
  • Coordinate referrals and exchange of records as needed with appropriate release of information.
  • Develop individualized, recovery-oriented treatment plans that promote overall physical and behavioral health well-being.
  • Collaborate with clients on goals for discharge and aftercare planning to support ongoing growth and change.
  • Maintain up-to-date medical records as specified by state/program regulations.
  • Complete all documentation within the electronic health record (EHR).
  • Participate in required in-house and appropriate out-of-house trainings.
  • Attend staff meetings and participate in formal group supervision and formal individual supervision as outlined by program regulations

What We're Looking For:

  • Master's or Bachelor's Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services)
  • Must have full certification as an addictions counselor or a co-occurring disorders professional by a statewide certification body such as: Certified Alcohol and Drug Counselor (CADC) Certified Advanced Alcohol and Drug Counselor (CAADC)
  • Reliable transportation for travel within the community.
  • Ability to work flexible, non-traditional hours based on the client's needs
  • Must obtain the following clearances prior to hire: PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate
