- Coordinate and oversee all direct and indirect patient services provided by clinical organization personnel.
- Provide supervision and guidance to clinical staff to ensure the quality of services and maintenance of standards.
- Perform chart audits.
- Gather data for QAPI and participate in QAPI activities.
- Appraise clinical staff performance during their probationary period and annually, provides teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff.
- Identify and evaluate patient needs and appropriateness of hospice care by reviewing the initial intake screening before assignment of direct care staff.
- Oversee the maintenance of patient clinical records and assure compliance with local, state, and federal laws as well as CHAP standards of care.
- Assist with the orientation of new staff.
- See patients in the home setting as needed.
- Participate in audits, IDG meetings and QAPI.
- Maintain adequate and appropriate medical supplies.
- Responsible for orientation of new personnel as well as in-services for current staff.
- Responsible for ensuring Medicare, State, and CHAP regulations are followed by all clinical staff.
- Assume responsibility for self-development by continually striving to improve Nursing/Management practice through formal education, attendance at workshops and conferences, active participation in professional and related organization meetings and/or individual research and reading.
- Other duties as assigned.
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and abilities required.
- Must be a graduate of an accredited School of Nursing.
- Must be a Registered Nurse with an active state license.
- Minimum of 2 years nursing experience required, hospice or skilled home care experience highly preferred.
- At least one year of supervisory experience required.
- Ability to market and deal tactfully with customers and the community.
- Knowledge of business management, governmental regulations, and CHAP standards.
- The ability to communicate well, both verbally and in writing.
- Must have the ability to demonstrate self-confidence and positive attitude toward self and others and maintain commitment and enthusiasm to goals achievement.
- Able and willing to travel as needed.

- Work for this position is performed in patient homes, care settings/facilities, and office settings. This position will have to navigate through a variety of different settings to provide care.
- There is exposure to varied home environments and potential infectious conditions.
