Job Summary
The TAP Transition & Employment Support Specialist provides transition, employment, and independent living support services to students and young adults with disabilities (ages 1425). This position supports the development and implementation of individualized transition and employment plans in collaboration with the TAP Transition Team and Iowa Vocational Rehabilitation Services (IVRS). Services are delivered in school, community, and workplace settings to promote successful postsecondary and competitive employment outcomes.

Essential Functions

- Assist in the development and implementation of individualized transition and employment plans, including IVRS Individual Plans for Employment (IPEs).
- Provide individualized job coaching and on-site training to support skill development and successful employment outcomes.
- Support students/job candidates in developing work readiness skills, including resume writing, job applications, interviewing, and onboarding.
- Assist with career exploration activities, including job shadows, work-based learning experiences, and postsecondary visits, as directed.
- Support development of independent living skills, including accessing community resources, transportation training, and completion of necessary forms and documentation.
- Monitor participant progress and collaborate with the TAP Transition Team and IVRS staff to adjust supports as needed.
- Develop task analyses and implement strategies aligned to individual needs and job expectations.
- Build and maintain collaborative relationships with students, families, staff, employers, and community partners.
- Communicate effectively with all stakeholders regarding participant progress and program expectations.
- Maintain accurate and timely documentation, including case notes, reports, and required data systems (e.g., Excel matrix, IRSS/IVRS systems).
- Assist with program coordination tasks, including scheduling, communication, and general clerical duties.
- Respond to challenges and crisis situations in a professional and timely manner.
- Transport students/job candidates to community-based instruction, job sites, and related activities as required.
- Promote a positive image of the TAP program and the Cedar Rapids Community School District.
Required:
- Valid driver's license and ability to transport students/job candidates
- Bachelor's degree in Education, Human Services, Psychology, or related field, or equivalent combination of education and experience
- Ability to work independently and as part of a team
- Strong organizational, communication, and interpersonal skills
- Experience working with individuals with disabilities
- Experience in transition services, vocational training, or job coaching
- Job Coaching Certification
- Knowledge of transition services, employment supports, and community resources
- Understanding of disabilities and appropriate accommodations
- Ability to work effectively with individuals from diverse backgrounds
- Ability to collaborate within a multidisciplinary team
- Ability to manage multiple tasks and meet deadlines
- Proficiency in computer applications, including Microsoft Excel and data management systems
- Knowledge of applicable federal, state, and district regulations
- Frequent travel within the community
- Ability to work in a variety of settings, including school, community, and job sites
- Flexible schedule based on program needs
- Ability to safely transport students/job candidates
