What you'll do:
Support Within a Private Residential Home
- Provide individualized assistance with personal care, daily routines, meals, household activities, appointments, community participation, and leisure activities.
- Help maintain a comfortable, respectful home environment where individuals are supported with dignity, privacy, and choice.
- Use person-centered practices and follow each individual's plan of care and IPP to support physical, emotional, and psychosocial well-being.
- Observe and communicate changes in condition, behavior, preferences, or support needs to the appropriate team members.
- Health, Safety, and Rights
- Support safety, infection control, proper body mechanics, skin integrity, and environmental cleanliness throughout the shift.
- Protect individual rights and promptly report suspected abuse, neglect, exploitation, rights violations, missing belongings, or environmental concerns.
- Assist with bathing, dressing, grooming, toileting, mobility, feeding, communication, and other needs identified in the individual's care requirements.
- Communication and Team Responsibilities
- Work collaboratively with nurses, supervisors, families, visitors, peers, and interdisciplinary team members.
- Complete required documentation and care records accurately and within expected time frames.
- Participate in meetings, in-service education, planning meetings, annual reviews, and other required training opportunities.
- What sets you apart:
- High school diploma or equivalent
- Must be 18 years of age with valid Kentucky or Indiana Driver's License and a clean driving record
Preferred Experience
- One year of caregiving, direct support, healthcare, residential care, or similar human services experience.
