Social Worker – Full Time

Schedule: Monday–Friday | 9:00 AM – 5:30 PM
Cedar Haven Healthcare Center is seeking a compassionate and dedicated Full-Time Social Worker to join our skilled nursing team in Lebanon, PA. This role is essential in supporting residents’ emotional, social, and psychosocial well-being while ensuring quality care coordination and smooth transitions throughout their stay.
Responsibilities
Complete social assessments and develop comprehensive care plans addressing residents’ emotional and medically related social needs
Participate actively in pre-admission and admission processes
Provide empathetic support and effective communication with residents and family members
Document behavioral, emotional, and psychosocial changes in accordance with regulatory standards
Offer individual and group support during adjustment periods, crises, and other times of need
Develop and oversee volunteer programs aligned with residents’ social service needs
Orient new residents and families to the center, ensuring a supportive transition into the facility
Qualifications
Bachelor’s Degree in Social Work preferred (or a related field from an accredited institution)
Knowledge of social service practices, procedures, laws, and regulations related to long-term care
Strong communication, documentation, and organizational skills
Ability to work independently while maintaining compassion and professionalism
