A DAY IN THE LIFE AS A SOCIAL WORKER
As a Cedar Haven social worker, you spend much of your time speaking directly with the resident and their responsible parties. Social workers help to gather vital information at admission that is used to determine the appropriate care for the resident.
Social workers are also involved throughout the resident's stay, to monitor their adjustment to the facility, and the help with any areas of concern. You are responsible for arranging the discharge plans, which involves contacting many local agencies to secure necessary home health services and durable medical equipment.
Overall, Social workers help to keep many things running smoothly and provide lots of emotional support to residents and families. By making an impact with MDS, admissions, and discharges, you help Cedar Haven provide the most high-quality care to our residents every day.
QUALIFICATIONS FOR A SOCIAL WORKER
• Bachelor's Degree in Social Worker preferred, or a BS Degree in a related field
• 1 year experience in Social Worker in a healthcare setting preferred

WORK SCHEDULE
