Care Coordinator

Job Title: Care Coordinator – full-time

Location:Glen Cove, NY (Travel throughout Nassau County required)

Hourly Rate: $27.30 per hour; 35 hours per week schedule

Schedule: Monday – Friday (some flexibility needed to meet caseload needs)

Position Overview:

The Charles Evans Center is seeking a compassionate and skilled Care Coordinator to join our expanding care coordination team. In this role, you will be a vital member of an interdisciplinary team dedicated to providing care coordination for adults diagnosed with severe mental illness, multiple medical co-morbidities, and/or co-occurring substance abuse disorders.

Key Responsibilities:

• Advocate for and support clients in navigating the healthcare system.
• Engage with community agencies and healthcare providers to ensure clients have access to necessary services.
• Facilitate wellness self-management and reduce emergency room visits and hospitalizations.
• Provide expert support and psychosocial/substance abuse interventions.
• Maintain a focus on client outcomes and best practices.
  
  
  

Qualifications:

• Associate's Degree with two (2) years of experience in the human services field; or Bachelor’s Degree with one (1) year of experience in the human services field.
• Clean and valid NYS driver’s license.
• Ability to travel to multiple locations during the day throughout Nassau County.
