Summary: Community Support Specialist is responsible for assisting with the coordination and implementation of events, programs, and services with the overall intent of enriching the lives of all Samano Flats residents. The Community Support Specialist actively participates in a team that serves as a liaison between supportive service partners, community service agencies and resident needs. The job will include receiving requests for information, making proper referrals to sources that can help with identified needs, planning and managing programs as designated by the supervisor, and the responsibility of keeping complete and accurate documentation.
Essential Duties and Responsibilities:
Conduct Needs Assessments
  • Perform comprehensive intakes to understand residents' current circumstances, identify immediate needs, and outline short-term and long-term goals for personal growth and stability.

Build and Maintain Relationships
  • Establish trust and positive relationships with residents through regular check-ins, active listening, and providing consistent support tailored to their needs.

Communication
  • Maintain effective communication with Property Management and service partners in day-to-day responsibilities and duties.

Provide Day-to-Day Support
  • Assist residents with everyday challenges, such as navigating resources, accessing essential services, and ensuring they feel supported in their daily lives.
  • Compliance Programs and Events: socials, arts and crafts, health and nutrition classes, financial planning classes, among others
  • Plan, organize, and facilitate life skills workshops, educational sessions, and support groups to promote self-sufficiency and personal development. Collaborate with staff and community partners to enhance program offerings.

Pantry Management
Oversee pantry operations, ensuring it is well-stocked, organized, and aligned with the dietary and cultural needs of residents, while maintaining accurate documentation and reporting.

Data Management and Documentation
Maintain accurate, timely, and detailed case notes in systems like One Drive or SharePoint. Document all resident touchpoints, progress, and outcomes to ensure accountability and track success daily.

Liaison with Case Managers and Collaborative Partners
Identify residents requiring more intensive support and coordinate meetings with case managers to ensure they receive appropriate assistance.

Crisis Intervention and Disaster Relief
Respond promptly to resident crises, providing immediate support, guidance, or referrals to specialized services as needed.

Advocacy and Referrals
Advocate for residents by connecting them to external resources, including healthcare, employment, education, and housing services, to help them achieve their goals.

Promote Community Engagement
Encourage resident participation in community events by distributing flyers, calendars, and knocking on doors. Create opportunities for residents to connect and build support networks.

Professional Development
Attending continuing education workshops, training, and events to stay updated on best practices, emerging trends, and effective strategies in community support and social services.

Flexible and Creative Approach
Adapt to the unique and evolving needs of residents, employing innovative and compassionate methods to address challenges and achieve program objectives.

  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
  • Social Services knowledge preferred
  • Bilingual in English and Spanish preferred
  • High school diploma or equivalent.
  • A positive attitude and optimistic approach to enriching the lives of a diverse population.
  • Strong customer service skills and a commitment to keeping a dignified and open environment.
  • Sound judgment, diplomacy and patience.
  • Excellent oral and written communication skills.

Physical Requirements
Must have the ability to lift moderately heavy objects up to 25lbs. and perform duties requiring extensive maneuverability and normal strength.

Transportation Requirements
Reliable transportation is always required. As timely travel is required with short notice, a personal vehicle is required. Millage is reimbursed.

Attendance and Punctuality
Attendance is an essential job function. This position requires the ability to flexible and non-traditional hours. It is extremely critical that individuals be able to work their scheduled hours on a consistent basis and, if necessary, overtime hours when required.
