Overview:

The CDC Foundation is seeking a mission-driven Lead Nurse to support the Kentucky Rural Health Transformation Program (RHTP). Kentucky’s RHTP launches five interrelated programs designed to build rural health infrastructure and provide sustainable, long-term improvements. This plan directly supports Kentucky’s rural counties while advancing statewide impact through innovation, technology-enabled care and strengthened workforce recruitment pipelines for both clinical and non-clinical staff. This strategy will help build a resilient, integrated and technology-forward health system across the Commonwealth. PoWERing (People who Work for Engagement and Resilience) Maternal and Infant Health is one of these programs and aims to expand timely prenatal and postpartum care by deploying telehealth-enabled maternal care teams who will serve maternity-care deserts and high-risk regions, to help mothers and infants receive seamless, high-quality support.

 

The PoWER Lead Nurse is a Registered Nurse (RN) who will serve as a key member of the PoWER Program Team and supports the design, implementation, and continuous improvement of the PoWER Program statewide in accordance with regulations set forth by the Kentucky Board of Nursing. The role functions as a liaison between the pilot site-based PoWER nurses and the PoWER Program Team leadership, helping translate program design into consistent and effective practices across pilot sites. The Lead Nurse will provide guidance and coordination to support effective implementation of PoWER nurse led activities at pilot sites, promote alignment with PoWER Program goals, and ensure consistent documentation and data collection to inform performance monitoring and program improvement.

 

Required Qualifications:

• Active Kentucky Registered Nurse license

• Bachelor of Science in Nursing (BSN)

• Basic Life Support (BLS) certification

• Minimum five years of nursing experience, including experience in maternal, perinatal, postpartum, newborn, women's health, or related settings.

• Minimum two years of nursing leadership, charge nurse, program coordination, or clinical supervision experience.

• Certification in perinatal nurse leadership at hire or within 18 months from hire

• Strong written and oral communication skills

• Microsoft proficiency

 

Preferred Qualifications:

• Master of Science in Nursing (MSN)

• Certification in maternal infant health (RN-OBC)

• Experience with rural health care

 

Responsibilities:

· Serve as the primary liaison between pilot site PoWER nurses and the PoWER Program Team.

· Participate in the design and ongoing refinement of the PoWER program, including nursing workflows, implementation approaches, and standard operating expectations.

· Partner closely with the Program Team’s Medical Director, Lead Doula, Lead CHW, and Peer Support Specialist to support coordinated design, implementation, and achievement of program outcomes.

· Coordinate with the pilot site PoWER nurses on a regular scheduled basis to support consistent application of the PoWER model.

· Conduct site visits with each PoWER team at least once per quarter to gather feedback and identify opportunities for improvement.

· Elevate barriers to care and implementation challenges identified by PoWER site nurses to the Program Team for resolution and follow-up.

· Use emotionally intelligent communication skills to support collaboration, shared problem solving, and alignment across teams.

· Collect, review, and analyze data from PoWER sites as appropriate.

· Support the development of ongoing education, guidance materials, and shared learning opportunities for PoWER nurses.

· Engage with relevant community partners and organizations as appropriate to support PoWER implementation.

· Ensure PoWER site nurses complete and document care planning activities at defined intervals.

This role is involved in a dynamic public health program. As such, roles and responsibilities are subject to change as situations evolve. Roles and responsibilities listed above may be expanded upon or updated to match priorities and needs, once written approval is received by both the CDC Foundation and Kentucky Department for Public Health in order to best support the Kentucky Department for Public Health in their public health programming.

This position is currently a hybrid position. CDC Foundation retains the right to make adjustments to this as required.
