Program Description: Catholic Community Services’, Drexel House, an 86-unit low-income housing tax credit program and 16 bed emergency shelter that employs the Housing First model for chronically homeless and disabled adults. The program will have 24-hour monitoring staff on-site supervised by the PSI as well as multiple housing stability case managers that will specifically provide supportive services to all residents and clients. The program utilizes Section 8 Project Based Vouchers from the Housing Authority of Thurston County (HATC) and Low-Income Housing Tax Credit (LIHTC) among other local dollars

 

Hope Village is a sanctioned micro home shelter that provides safe shelter for adults experiencing homelessness in Olympia. CCS strives to provide a safe and respectful environment where program participants can access an array of emergency and supportive services and work towards more permanent housing. The Case Manager’s primary responsibility is to partner with the Participants at Hope Village to create individualized service plans that are focused on gaining housing, gaining or increasing income, obtaining medical/mental health/substance use supports and gaining other resources.

 

 

Position Description: The Case Manager’s primary responsibility is to provide housing stabilization services for shelter clients and permanent housing residents living at the Drexel House Campus.  This position works specifically with the “most vulnerable” previously homeless adults in Thurston County (as rated by the HEAT tool).  These services include, but are not limited to: housing relocation, eviction prevention, housing search, landlord liaison activities, financial coaching, housing counseling, connecting to resources, obtaining benefits, finding employment, crisis intervention & management, etc. This position will work closely with the Program Supervisor sand Resident Coordinators to implement supportive strategies aimed at ensuring housing stability for the clients of the shelter and residents of the permanent housing units. This position will take a vital role in fulfilling our agency’s goals of safety, stability, and supportive connections.

Responsibilities

MAJOR DUTIES AND RESPONSIBILITIES

  • Conduct strengths-based assessment of housing barriers, and work in partnership with residents to develop Housing Stability Plans for obtaining and maintaining permanent housing.
  • Work in partnership with clients, staff and community supports to develop strategies and short- and long-term goals for increasing income and obtaining and maintaining employment for employable residents.
  • Provide strengths-based case-management and advocacy to clients to support forward progress on goals aligned with their individualized Housing Stability Plans.
  • Conduct coaching sessions with clients (both individually and in groups) that include helping them to understand housing requirements, create and adhere to budgets and build other skills needed for independent living.
  • Assist residents with accessing resources in the community.
  • Maintain knowledge of Thurston County resources and provide appropriate referrals (using a warm-handoff method) to address barriers to stable housing and employment (i.e., medical, mental health, chemical dependency, domestic violence, education, immigration, etc.).
  • Coordinate services with other Drexel House staff, including Resident Coordinators and other Case Managers for complementary services or in a co-case management model.
  • Participate in multidisciplinary case staffing meetings as needed to coordinate services for clients (this includes external meetings as needed).
  • Track participant activities and ensure timely and accurate collection of program data as outlined by management team.
  • Collect and input required client data into HMIS, Sharepoint and Microsoft Teamsas required
  • Assist team members with ongoing continuous quality improvement to enhance program operations, including problem solving, helping to develop program systems or procedures, or participating in team workgroups.
  • Act as a primary staff person to support clients with situations and  behavior that puts client’s housing at risk, etc.
  • Assist clients with transportation or transportation resourcesto medical, mental health, chemical dependency appointments, etc.
  • Develop and implement group educational series, community building events, etc.

  • Attend trainings as directed by supervisor.
  • Other duties as assigned.

Qualifications

Minimum Qualifications

  • Post-Secondary education in social services and 1-year experience providing direct services to low-income/no-income households in crisis, or 3 years’ experience providing direct services to low-income/no-income households in crisis. 
  • Detail oriented and can perform organizational duties with a high level of accuracy.
  • Demonstrate ability to manage and educate.
  • Demonstrate quality communication and listening skills.
  • Ability to support and maintain an environment that honors cultural awareness and competency.
  • Ability to support the missions set forth by each program and their parent organizations CCS/CHS as well as the mission of the Catholic Church.
  • Willingness to learn and work well in a team environment.
  • Must have reliable transportation, valid driver’s license, and automobile insurance.
  • Must have an acceptable driving record per CCS’ driving policy. (required to provide current driving abstract).
  • Proof of negative TB test within past 12 months.
  • Applicant must successfully pass required background checks prior to an offer of employment.

Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural situations.

 

Preferred Qualifications

  • Masters degree in a psychology, social work, or related field.
  • 3-5 years’ experience with individuals experiencing homelessness.
  • Experience organizing schedules.
