Position Summary:
The Residential Services Case Manager will provide comprehensive case management services to residents of St. Vincent Apartments, including a client-centered case management plan to meet resident’s health and wellness needs. The Residential Services Case Manager will also provide appropriate referrals to community services including medical, mental, dental, and/or substance abuse providers. The Residential Services Case Manager will conduct wellness checks and organize activities for the residents on a regular basis.
Essential Responsibilities:
- Meet one-on-one with residents to assess needs and connect them with critical resources and information on assistance that offer them social, financial, educational, and health-related benefits.
- Work to provide residents with activities and events, like game and movie nights, food drives, donation events, and educational workshops from local partners, to keep them engaged with one another and the community.
- Work closely with Case Management to create seamless transitions to and from resources.
- Maintain awareness of potential resources in the community and make proper referrals.
- Work with Case Managers to assess and identify resident’s eligibility to various housing, employment, substance use, nutritional assistance, or other applicable programs available in the community, and provide appropriate program guidelines to residents.
- Complete required program related reporting and documentation, utilizing a range of computer programs and data collection systems.
- Utilize electronic records to document and track all case management services and referrals.
- Protect all confidential information, company property and electronic data. Comply with safety rules.
- Other related duties as directed.
Knowledge, Skills and Abilities:
- Must be computer literate and possess effective written and verbal communication skills.
- Must be able to work independently or with a group.
- Must have a strong sense of focus and must be task-oriented and non-judgmental with a clear sense of personal boundaries.
- Must have a respect for confidentiality involving both residents and employees.
- Must be able to work in a variety of settings with culturally diverse groups, and be culturally sensitive, as appropriate.
- Must be able to manage personal appointments and schedule, prioritize work, and meet necessary deadlines.
Qualifications:
- Prior full time professional-level casework experience in a social service agency is preferred.
- Prior experience with the homeless population is preferred.
- Flexible schedule to work after normal office hours and weekends.
- Regular schedule will include either Saturday or Sunday each week.
Physical Requirements:
- Primarily works in the St. Vincent Apartments, but may also work throughout the agency and off site.
- Ability to regularly communicate with residents, co-workers, and supervisors.
- Ability to operate and use office equipment, especially computer, keyboard, and mouse.
- Ability to continuously sit and/or stand.
- Ability to walk continuously if needed, and climb stairs as needed.
- May occasionally lift up to 25 pounds.
This position pays $20.00 per hour.
