Job Summary:
The Nursing Coordinator is responsible for overseeing schedules for the clinical department, assisting in management of clinical documentation, and oversight of one of our shared aide office settings.
Responsibilities:
Oversight and management of one of CCORs shared aide building offices Monday through Friday mornings.
Meet with potential clients and/or families when onsite at shared aide sight and gather and send pertinent information to Intake team for CCOR.
Tracking and management of medical orders sent to physician offices for review and signatures per NYSDOH regulation.
Communicate with hospitals, rehab centers, and providers to monitor clients admitted to an alternate setting to track potential return to CCOR services.
Scheduling nursing visits in AlayaCare to ensure maximization for billing and appropriate utilization of services, and clinical support tasks as needed.
Pre-billing readiness for clinical staff visits.
Perform when needed agency Meet and Greets to ensure appropriateness and ability to provide a safe plan of care in the Shared Aide setting.
Successful completion of CCOR Personal Care Aide Training program.
Perform Clients personal care and/or assist with activities of daily living as needed.
Classroom support with some training under the direct supervision of an RN.
Perform other duties as assigned.

Position Requirements:
Previous home care experience preferred.
Scheduling experience preferred.
Ability to successfully complete a Competency & Proficiency Test and Skills Evaluation appropriate to job title as part of this role.
Excellent verbal and written communication skills.
Excellent human relations and customer service skills.
Ability to function with minimum direct supervision, to make decisions, to solve problem problems, and to manage emergencies.
Ability to relate to diverse and multi-cultural populations.
Successful completion of a background check.
Ability to lift up to 25 pounds and to sit, stand, bend, stoop, kneel, pull, climb, reach, and perform repetitive movements of the upper extremities.
Pass a physical examination and immunization requirements after being offered employment and complete a health assessment annually, thereafter.
Possess a current, valid New York State drivers license and a reliable vehicle.
Completion of CCORs PCA certification and orientation program
