What are the responsibilities and job description for the Case Manager position at CCN Pharmacy?
Essential Functions & Responsibilities
§ Perform outreach activities to assigned patients, develop working relationships with referral agencies, and work closely with Pharmacy staff to ensure timely management of patient’s prescription needs.
§ Schedule and perform new assessments and reassessments.
§ Provide knowledge of Medicaid managed care, available resources, how to access providers, and the benefits of utilizing specialized pharmacy programs.
§ Conduct comprehensive health risk and medication assessments of identified patients, ensuring documentation is accurately and consistently completed on time.
§ Conduct additional assessments as indicated by disease state.
§ Develop recommended care plans and link patients to appropriate services based on information gathered from profiles, health risk assessments, and medication assessments.
§ Schedule follow-up with patients as needed to ensure linked resources were effective, updating care plan as indicated.
§ Maintain detailed electronic and written records and database of resources.
§ Monitor effectiveness of care plan and progress made by patient, updating annually when completing the re-assessment.
§ Coordinate with outside agencies, including the Department of Public Welfare, to ensure access to medical benefits for patients.
§ Act as a liaison with medical providers.
§ Implement and adhere to the policies and procedures for case management program.
§ All other duties assigned by supervisor.
Job Type: Full-time
Pay: $22.00 - $24.00 per hour
