What are the responsibilities and job description for the Full Time Case Manager position at CBH Care?
- Provides outreach services to consumers with a history of mental illness in imminent risk of becoming homeless, as well as those who are currently homeless or residing in shelters.
- Manages assigned caseload and continuously evaluates consumer’s service needs
- Assists consumers in obtaining access to medical, social, rehabilitative, financial, housing and mental health services, in addition to any other qualifying services
- Maintains accurate case records until consumer is transferred or discharged from program
- BSW in social work, BA/BS in psychology, or a related field
- Two years of previous experience in mental health and human services
- Valid Driver’s License
Pay- $41,500 annual salary
Shift- Monday through Thursday, 9am-5pm, Friday, 9am-4pm
