Community Support Services

Responsibilities

• Provides IMR and crisis intervention services to assigned consumers on caseload
• Assists consumers with financial management and continuous linkage to community resources and benefits
• Coordinates medical appointments, assists with activities of daily living skills, and meeting program guidelines
• Implements prescribed treatment plan and assists in the development of IRPs
• Must be able to work independently and travel to consumers’ residencies as required

Qualifications

• BSW in social work, BA/BS in psychology
• BA/BS in a related field with two years of experience in mental health
• Valid New Jersey driver’s license

Pay:

• $19.50 per hour
