What are the responsibilities and job description for the Community Support Services position at CBH Care?
Responsibilities
- Provides IMR and crisis intervention services to assigned consumers on caseload
- Assists consumers with financial management and continuous linkage to community resources and benefits
- Coordinates medical appointments, assists with activities of daily living skills, and meeting program guidelines
- Implements prescribed treatment plan and assists in the development of IRPs
- Must be able to work independently and travel to consumers’ residencies as required
Qualifications
- BSW in social work, BA/BS in psychology
- BA/BS in a related field with two years of experience in mental health
- Valid New Jersey driver’s license
