As a Licensed Practical Nurse (LPN) at Cayuga Ridge Nursing & Rehabilitation Center, you will have the opportunity to make a meaningful difference in the lives of our residents. Our 144-bed Skilled Nursing & Rehabilitation Center is located on the beautiful West bank of Cayuga Lake in Ithaca, NY, and we are proud to offer a comprehensive range of services to meet the unique needs of each individual.
About the Role:
Provide high-quality nursing care to our residents, focusing on their physical, spiritual, and emotional well-being
Work collaboratively with our interdisciplinary team of healthcare professionals to develop and implement individualized care plans
Administer medications, perform wound care, and manage chronic conditions in accordance with established protocols
Communicate effectively with residents, families, and healthcare providers to ensure seamless transitions and care coordination
Responsibilities:
Provide direct patient care
Assist with activities of daily living (ADLs)
Report changes in resident condition to the nursing team
Maintain accurate and up-to-date records of patient care and treatment
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
