Duties
- Conduct initial intake interviews with families to gather essential information regarding their circumstances
- Assess urgent needs related to elder care services.
- Develop safety plans and coordinate with social workers, legal professionals, and community resources to ensure a plan is in place.
- Document all interactions thoroughly in case management systems, maintaining accuracy and confidentiality
- Provide emotional support and guidance to clients navigating complex family law or conflict management issues
- Facilitate connection with the specialists on our team.
- Monitor ongoing situations and follow up with clients to ensure continued safety and access to resources
- This role will be managing some office support systems as well.
Qualifications
- Proven experience working in social work, or related fields such as home health, hospice or hospital care management or social work.
- Strong understanding of crisis intervention techniques and crisis management strategies
- Knowledge of conflict management principles
- Excellent communication skills—empathetic listening combined with clear documentation practices
- Ability to remain calm under pressure while managing sensitive situations involving families in crisis
- Familiarity with social work practices, behavioral health assessments, and trauma-informed care approaches
- Bachelor’s degree in social work, psychology, human services or a related field is preferred; relevant experience may substitute for formal education
- Autonomous worker with above average follow through.
