Placement Navigator (Full-Time)

Position Overview
We are seeking a Placement Navigator to support families through the senior care placement process while building relationships with healthcare partners in the community.
This role is ideal for someone who is comfortable in the field, enjoys working independently, and can balance empathy with professionalism in emotionally sensitive situations.
Key Responsibilities
- Meet with families to assess care needs, preferences, and goals
- Educate and guide families through senior living options (AFH, Assisted Living, Memory Care)
- Coordinate and attend community tours with clients
- Build and maintain relationships with hospitals, skilled nursing facilities, and referral partners
- Manage placement process from initial intake through successful move-in
- Maintain accurate documentation and communication in company systems
- Represent CayCare professionally in both family and referral partner interactions
Qualifications
Required:
- Knowledge of medical terminology
- Ability to work independently and manage a flexible schedule
- Strong communication and interpersonal skills
- Comfortable with local travel throughout Pierce, King, and Kitsap Counties
- Valid driver’s license and reliable transportation
Preferred:
- Experience in home health, hospice, case management, or senior care
- Background in healthcare, social services, or related field
- Experience working directly with families in care planning or crisis situations
