What are the responsibilities and job description for the Substance Use Disorder Health Home Coordinator position at Catholic Social Services of the U.P.?
SUMMARY:
The role of Community Health Worker/Peer Recovery Support assists individuals in understanding and accessing substance use disorder services and offer support, guidance, and share personal experience with individuals in the early and ongoing recovery process. They act as an intermediary between the consumers and providers of healthcare and social services. They identify health-related issues, collect data, discuss consumers’ health concerns within the community with proper release of information. They promote wellness by helping people adopt healthy behaviors, implement programs, and advocate for people who have limited access to health resources and social services. They model competency in recovery and on-going coping skills utilizing personal recovery experience to mentor, encourage and inspire hope in the consumer.
RESPONSIBILITIES:
- Engages individuals in a welcoming manner and use personal recovery experience to facilitate the recovery process.
- Provides individuals with culturally appropriate health and information about services and how they are delivered.
- Assists individuals and families in understanding recovery, including the use of a care plan and how a variety of services can be helpful in meeting recovery goals, including access to recovery support services.
- Creates a care plan identifying social determinant of consumers’ needs.
- Provides consumers with information about local drop-in centers, peer support groups, 12 step programs and other recovery-based programs and activities and community resources/services and assist participants in transitioning to and between provider and community services.
- Maintains appropriate boundaries with individuals served. Develops positive, professional relationships with consumers and other staff, and respects the boundaries of clinical roles.
- Collaborates with other recovery coaches and peer support specialists to maintain and develop resources for community use such as resource guides. Ensures resource guides are updated as needed.
- Utilizes strength-based, trauma informed techniques when creating care plans.
- Assists program participants with acquiring resources that assist them with basic needs (food, clothing, shelter, bathing, etc.), developing life skills and employment readiness, and in finding decent, affordable housing of their choice.
- Aids participants in completing applications, obtaining childcare, and providing or arranging transportation as it pertains to the client’s needs, to mitigate transportation barriers.
- Participates in and leads program meetings, takes and maintains meeting minutes.
- Participates in staff training activities, completes all required training and continuing education opportunities.
- Documents all attempted and completed interactions with program participants in progress notes.
- Remains in compliance with organizational policies and procedures.
- Responsible for ensuring all intake paperwork is completed correctly per funding requirements.
- Responsible for entering all required data into EMR to bill for services.
- Maintains information on program participants and contacts/meetings/etc.
- Serve as a personal guide and mentor for consumers in recovery and promote recovery by assisting in the removal of barriers and obstacles to recovery.
- Maintains a high level of confidentiality
- Sees Catholic Social Services of the U.P. in its function as a ministry of the Church to the people of the Upper Peninsula.

QUALIFICATIONS:
- High school diploma/GED required.
- Hold a Peer Recovery Coach certification from the Connecticut Community for Addiction Recovery or the Michigan Department of Health and Human Services, required.
- Ability to use a computer and other office equipment proficiently. Knowledge of Microsoft Office programs and ability to use or learn databases and electronic medical records.
- Strong written and verbal communication skills.
- Strong interpersonal skills required, including the ability to deal with individual in emotional or crisis situations.
- Ability to maintain a high degree of confidentiality and maintain professional boundaries with program participants.
- Ability to multi-task, manage time, rearrange priorities, and manage frequent change, delays or unexpected events.
- Ability to manage self-care in personal recovery.
- An awareness and sensitivity of the particular cultural aspects of the population in the local area. For example, children, the elderly, Native Americans, etc.
