Position Summary
Life Care Inc. is seeking a qualified and dedicated Housing Specialist to support individuals enrolled in the Mental Health Rehabilitation Services (MHRS) program. This role is responsible for assisting eligible individuals and families in securing and maintaining safe, stable, and affordable housing.
The Housing Specialist serves as a liaison between clients, housing authorities, and community partners, while ensuring compliance with program requirements and supporting clients with behavioral health needs.
Responsibilities
- Coordinate and manage referrals from agencies such as the Department of Behavioral Health (DBH), Federal Bureau of Prisons (FBOP), Saint Elizabeths Hospital, and other federal and community organizations
- Conduct initial screenings to determine client eligibility based on program and referral criteria
- Collect, review, and maintain required documentation, including identification, clinical records, CRF packages, SPDAT assessments, and Community Partnership applications
- Assist clients with placement into appropriate housing options, including SAFE housing environments and recovery-based housing programs (e.g., Oxford House)
- Conduct home visits and housing inspections to ensure compliance with health and safety standards
- Develop and maintain relationships with landlords, housing authorities, and community partners
- Coordinate care and services with external providers to support client stability
- Provide referrals to additional support services, including behavioral health, legal, and temporary housing resources
- Conduct ongoing monthly client visits to monitor housing stability and provide continued support
- Stay informed on local housing regulations and advocate for client needs
Required Qualifications
- Bachelor’s degree in Social Work, Behavioral Health, or a related field
- Experience working with at-risk or vulnerable populations
- Knowledge of housing resources and services for individuals with serious mental illness
- Understanding of Permanent Supportive Housing and evidence-based practices
- Strong organizational, communication, and case management skills
- Ability to handle crisis situations, including eviction prevention and homelessness intervention
- Valid driver’s license and reliable transportation
Preferred Qualifications
- Prior experience in mental health or community-based services
- Bilingual skills (highly desirable)
- Familiarity with DBH systems and housing programs
Physical Requirements
- Prolonged periods of sitting and working on a computer
- Ability to travel locally for home visits and meetings
- Ability to lift up to 15 pounds as needed
