Night Shift - 9p - 7a

Pharmacists fill orders for parenteral and enteral medications, monitor patient drug therapies, and provide drug information.  Staff pharmacists supervise and direct support personnel.  In the absence of the pharmacy manager/supervisor, a staff pharmacist may be required to assume the essential responsibilities and perform the duties of the pharmacy manager.

Responsibilities:

• Demonstrates accuracy in dispensing of all medications.  Compounds and dispenses pharmaceuticals, including intravenous admixtures, chemotherapy, and parenteral nutrition products accurately.
• Issues controlled substances to patient care areas and maintains records as required by law.
• Monitors drug therapy regimens for contraindications, drug-drug interactions, allergies, and appropriateness of drug and dose.
• Reads, extracts, and interprets information in patient charts accurately.
• Detects and reports suspected adverse drug events accurately in a timely manner.
• Sustains the formulary by minimizing non formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
• Provides clinical consultation and clarification to practitioners.  Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.
• Provides accurate drug information to professional staff.
• Provides drug information to patients and families when needed.
• Documents all clinical activities and interventions accurately and completely.
• Participates in quality improvement and medication use review activities of the department.
• Readily participates in orientation/training of new employees as directed.
• Works independently with minimal supervision.  Organizes and prioritizes work assignments.  Ensures that pharmacy services are provided in a timely manner.
• Completes and documents monthly inspections of all assigned medication storage areas in a timely and comprehensive manner.  Inspection assignments are completed without supervisor intervention.

Requirements:

• Graduate of an ACPE-accredited School of Pharmacy with a BS in Pharmacy, an MS in Pharmacy or a Doctor of Pharmacy.
• NYS Pharmacy license required.  Oncology pharmacy board certification preferred (BCOP).
• Basic and advanced computer skills; proficient in the use of Microsoft Word, Excel, and PowerPoint.  Able to maintain concentration and accuracy despite a busy environment.
• Outstanding oral and written communication skills. Able to work collaboratively on a multidisciplinary team.
• Health system pharmacy experience preferred but not required.

The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
