Summary:

The Clinical Stroke Coordinator for Accredited Primary Stroke Center (PSC) is responsible for the overall Acute Stroke Program, assuring quality patient care. The PSC Stroke Coordinator is responsible for the oversight of stroke patient coordination at PSC site at Kenmore Mercy Hospital as well as Mount St. Mary's Hospital(MSM) and Lockport Memorial Hospital(LMH). The Stroke Coordinator works in collaboration with the Neuroscience Service Line, Medical Director of Stroke and Nursing Administration to continually develop facility Stroke Program in conjunction with system initiatives. The Stroke Coordinator provides leadership to the facility's stroke program and facilitates the bedside clinical care of Stroke/TIA, and other complex cerebrovascular patients to assure that all patients receive comprehensive and timely treatment interventions that are consistent with evidence based care as defined by Clinical Practice Guidelines. Assists in furthering clinical standardization goals through participation in multi-disciplinary teams to develop order sets, standing orders and care algorithms. Collaborates with all members of the healthcare team via consistent clinical patient rounds to assist in the planning, implementing, monitoring and evaluation of care delivery for the stroke/TIA/complex cerebrovascular patients within the stroke system of care. The Stroke Coordinator provides formal and informal education to staff and community related to cerebrovascular-specific topics, and acts as a resource for staff, physicians, and community to support Stroke program goals. The Stroke Coordinator position facilitates stroke system of care, collaborating with all CHS sites, nursing and physician leadership and Education to develop and facilitate educational programs to enhance the staff's knowledge of current trends in stroke care.

Develops, implements and maintains a quality improvement process for the stroke patients. Collaborates with and assists the multi-disciplinary team in developing and implementing projects that enhances the quality of the hospital for stroke. The role is one of expert practitioner, collaborator, consultant, educator, clinician, researcher and change agent.

The stroke coordinator must develop a strong professional report with the medical directors of the stroke programs within CHS.

Responsibilities:

EDUCATION
  • Bachelor of Science degree in Nursing required
  • A Master of Science degree is preferred.
  • Registered Nurse must maintain professional License in the State of New York.
  • Preferred certification as Certified Neuroscience Registered Nurse (CNRN)
  • National Institute of Health Stroke Scale (NIHSS) certification within 90 days of position assignment, maintained per organization policy thereafter.
  • Modified Rankin training within 90 days of the position assignment, maintained annually thereafter.
  • Eight (8) hours of stroke education within 90 days of position assignment, maintained annually thereafter.

EXPERIENCE
  • Minimum of three (3) years recent clinical nursing experience
  • BLS and ACLS must be obtained within 12 months of assuming position and maintained throughout.
  • Clinical experience in Neuroscience / Stroke preferred

  • Planning skills to develop goals and objectives in concert with those of the hospital and implements action plans to meet those goals
  • Analytical ability to identify opportunities for improvements as well as positive problem resolution
  • Teaching abilities to promote optimum orientation and continuing education of staff
  • Has current knowledge of health care trends and issues and relates same to hospital and work area environment
  • Performs principal clinical responsibilities, according to the appropriate age and condition of the patient, as specified in the general nursing care competencies and unit specific competencies
  • In depth knowledge of change theory and demonstrated ability and guide change process
  • Must have strong verbal and written communication skills
  • Must have strong time management and organizational skills
  • Must have strong customer service and conflict resolution skills
  • Must possess strong computer skills

WORKING CONDITIONS:
Subject to inside environmental conditions. Must be able to work in hectic environment making sound decisions at all times. Work is demanding and stressful at times. Visual and auditory acuity are essential. Considerable walking and standing during the tour of duty. Knowledge and utilization of proper body mechanics is essential as lifting, pulling and pushing patients and equipment are required. Works indoors in a well lighted, well ventilated area. May be exposed to communicable diseases. Travel between KMH, MSM and LMH is required as needed.
