Summary:
The Transitional Care Social Worker provides comprehensive social work and discharge planning services to sub-acute and long term care patients. Working as a member of the interdisciplinary healthcare team and in close collaboration with the Sub-Acute Assistant Director of Nursing, Rehabilitation Manager and Director of Social Work, as well as the Director of Rehabilitation for Senior Services, the Transitional Care Social Worker utilizes technical skills, critical thinking, and interpersonal skills to develop and coordinate a needs based plan of care.

Responsibilities:
EDUCATION
- Bachelor's Degree in Social Work or related field required; and
- Must have and/or attain Screen certification within 6-9 months
- NASW Certified Social Worker Case Manager (C-SWCM) preferred
EXPERIENCE
- Minimum 3 years prior experience in healthcare setting, long term care, acute care, or rehab setting required
KNOWLEDGE, SKILL AND ABILITY
- Must be able to read, write and understand the English language;
- Must be able to make independent decisions when circumstances warrant such action;
- Excellent Interpersonal networking and advocacy skills;
- Excellent critical thinking, discernment and psychosocial assessment skills;
- Able to develop and maintain relationships with multiple health care and service providers across the continuum of care;
- Able to develop, implement and evaluate a needs based plan of care;
- Subject to call back in the event of an emergency;
- Communication skills which may be inclusive of approach, tone of voice, speed of communication, respectfulness and non verbal communication; and
- Microsoft Office/Computer Experience
WORKING CONDITIONS
- Prolonged periods of walking, and sitting;
- Hours may vary/ irregular;
- Travel between CH facilities may be required;
- NYS License & must have own transport or means of transport to other CH locations;
- Clinical Setting;
- Frequent Interruptions; and
- Professional Appearance for all public interactions
- Normal heat, light space, and safe working environment typical of most office jobs; and
- Occasional exposure to one or more mildly unpleasant physical conditions
