Summary & Objective
- The Speech Language Pathologist is responsible is to provide direct and indirect speech language therapy services primarily on a visiting and intermittent basis in the patient’s home in accordance with the policies and procedures of Catholic Home Health Services of Broward.
- Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
- Participate in the development and periodic review of the Plan of Care (POC).
- Performs assessments and treats disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders and conditions.
- Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening.
- Observe, record and report changes in the patient's condition and response to treatment to supervisor and/or the physician.
- Provide instruction and training to patients in use of alternative communication systems when appropriate.
- Provide counsel and instruction to patients, families and healthcare staff.
- Participate in care coordination activities and discharge planning.
- Apply concepts of agency’s infection control plan and universal precaution in coordination/performing client cares activities to protect both patient and staff (OSHA).

- Must be able to lift and/or move up to 50 pounds and push/pull up to 250 pounds, walk, climb stair or ladders, stand on feet for extended periods of time, etc.

- Clear and Active Speech Language Pathology license to practice in the state of Florida.
- CPR certification.
- Bachelor’s or Master’s Degree in Speech-Language Pathology.
- 2 years clinical experience preferred/
- CCC required or currently completing CFY to obtain CCC.
- 1-2 years of experience with a Medicare Certified Home Health agency preferred.
- Knowledge of AHCA regulations, Medicare COP’s guidelines required.
- Must have knowledge of computer office/clinical software.
- Must be able to read, write and understand the English language.
