Catholic Guardian Services is actively interviewing for Care Managers. We provide Care Management Services to children in our foster care program and in the community through NYS Children's Health Home Program . The Care Manager coordinates and connects children to needed medical, mental health, educational and social services. Care Manager education & qualifications: BS/BA or Masters with 1 year experience. Prior case management, child welfare, health care or mental health experience a plus. In-office 2 days a week (3 days during orientation period). Supportive work and learning environment. Benefits include four weeks PTO, all federal and other observed agency holidays, tuition and student loan assistance & 403B contribution. Depending on experience Bachelors $52,000-$55,000; Masters $57,000-$61,000. Office located on Grand Concourse, Bronx, NY.
