Primary Function of Position

As the After-Care Coordinator, your primary responsibility will be to provide essential direct services to young adults who have transitioned from the Refugee Foster Care Program, focusing on those who have aged out or reunified with family. Your role encompasses meticulously tracking outcomes, establishing crucial connections to community resources, and facilitating the development of vital life skills for Unaccompanied Refugee Minors. It is imperative that all services you provide strictly adhere to state and federal regulations, align with the established policies and procedures of the Refugee Foster Care program, and actively contribute to fostering the self-sufficiency of the youth you serve.


Major Position Responsibilities

  • Survey Distribution and Analysis: Distribute electronic surveys to youth one year, three years, and five years after terminating services with CCS. Analyze survey data to gain insights that inform and enhance services provided by the Refugee Foster Care program. Responsively address needs identified in the survey.
  • Resource Information and Access: Inform After-Care youth about ongoing resources CCS offers, such as Educational Training Vouchers and Chafee Funds. Assist them in the application process and ensure access to these essential funds.
  • Community Resource Connection: Actively connect After-Care youth to various community resources, including public benefits, physical and mental health services, educational opportunities and funding, housing options, and immigration services.
  • Home Visits and Reporting: Conduct home visits and generate annual reports for the Office of Refugee Resettlement for After-Care youth who have been reunified with family, ensuring compliance with reporting requirements.
  • Transition Preparation: Prepare youth currently in the Refugee Foster Care program for aging out by conducting meetings six months before this transition. Complete a comprehensive assessment of their life skills and provide recommendations to youth, staff, and foster families based on identified needs.
  • Life Skills Curriculum Instruction: Deliver engaging Life Skills curriculum sessions to youth, employing a hands-on approach. Based on observed trends among emancipated youth, offer feedback and suggest improvements to enhance the curriculum's effectiveness. The curriculum covers various topics, including education, career planning, housing, money management, physical and mental health awareness, public transportation, communication, and legal aspects.
  • Stay Informed on Opportunities: Stay abreast of academic, vocational, and employment opportunities to guide and support After-Care youth effectively.
  • Outreach and Contact Updates: Conduct proactive outreach to After-Care youth to update their contact information, ensuring accurate and up-to-date records.
  • Meeting Participation: Attend and actively participate in weekly Refugee Foster Care staff meetings and monthly Transition to Adult Living team meetings.
  • Flexible Support: Perform other duties as assigned.

Work Environment

  • This role is in a professional office environment, utilizing standard office equipment, including computers, phones, photocopiers, filing cabinets, and fax machines.
  • Some local travel is anticipated as part of the role's responsibilities.

Physical Demands

  • The nature of this position is primarily sedentary, with occasional filing tasks that involve lifting files, opening filing cabinets, and, as needed, bending or standing. The role requires the ability to sit and stand for extended periods, walk moderate distances, and occasionally participate in physical activities to assist youth, including carrying groceries or household supplies.
  • Driving is an essential requirement for this position.

  • Position-Related Competence: Possesses the ability to perform functions outlined in the nature and scope of the position.

Education, Training, and Experience

  • Educational Background: An Associate's degree or higher in social work or a related field from an accredited institution is preferred.
  • Alignment with Mission: Familiarity with and support for the mission of Catholic Community Services of Utah and the vision of the Catholic Church as articulated by the diocesan bishop.
  • Lived Experience: Experience in the Refugee Foster Care program is strongly preferred.
  • Multi-cultural and Refugee Experience: Experience working with multi-cultural and refugee communities.
  • Independent Living Skills: Prior experience conducting independent living skills training with youth.
  • Advocacy Experience: A history of advocating for young adults in various capacities.

Background Screening Requirement

  • Must pass a national BCI/FBI background check.

Licenses, certifications, and credentials

A current Utah driver's license and provide proof of auto insurance meeting minimum policy liability limits of $50,000 per person and $100,000 per incident. Additionally, be 21 years of age or older, maintain a clean driving record, and have the capacity to fulfill CCS vehicle safety standards.

Position status and expected schedule

  • Position Status: This full-time position requires 40 hours of work per week.
  • Work Hours: The 40-hour workweek is typically distributed between Monday and Friday, with operational hours falling between 8:00 AM and 9:00 PM.
  • Weekend On-Call: A rotating weekend on-call schedule is part of the role, occurring 3-4 times a year.
