General Description

Under the supervision of the Housing Supervisor, the Cross trained Housing Case Manager (HCM) is responsible for coordinating and delivering resident-centered services across both the Supported Housing Program and the Treatment Apartment Program. HCM provides education, support, and accountability to ensure residents receive the services they need while maintaining compliance with all contractual, regulatory, and agency requirements. HCM delivers direct services including assessments, individualized service planning, goal development, and skill-building. The position supports residents in developing activities of daily living (ADL), communication, and self-advocacy skills, while assisting them in managing housing stability, accessing community resources, and achieving personal goals.

Essential Duties And Responsibilities

  • Receives cross training in both the Supported Housing Program and the Treatment Apartment Program to ensure continuity and flexibility of services.
  • Provides support across programs as needed, including participation in two-person visits, crisis response, and coverage during staff absences.
  • Conducts resident assessments and develop, implement, and review individualized service plans and goals.
  • Provides direct services and support to residents including skill development in ADLs, communication, and self-advocacy.
  • Facilitates activities, and opportunities that promote resident engagement and independence.
  • Assists residents in accessing community-based services, benefits, entitlements, and other funding resources.
  • Advocates for residents and mediate issues related to landlord/tenant concerns, service providers, employment, or funding barriers.
  • Assists residents in meeting the obligations of tenancy, including timely rent payment and addressing delinquency issues when necessary.
  • Supports residents in maintaining engagement with treatment providers to ensure continuity of services.
  • Maintains accurate and timely case file documentation in accordance with program regulations, funding requirements, and agency policies.
  • Completes required documentation including assessments, service plans, service plan reviews, utilization reviews, progress notes, hospitalizations, discharge planning, and income verification.
  • Submits resident files to the Housing Supervisor within 28 days of admission for audit and compliance review.
  • Renews and maintain time-sensitive documents including Releases of Information (ROI), Consents to Release Information, Physician Authorizations, Housing Quality Inspection packets, Tenant Calculation Worksheets, and Income Verification documentation.
  • Completes required medication monitoring training and support clients in licensed programs by overseeing and guiding the safe self-administration of medications.
  • Participates in regular supervision meetings with the Housing Supervisor to review documentation and ensure compliance with program standards.
  • Conducts routine housing inspections to monitor resident well-being and identify areas where support is needed.
  • Works collaboratively with residents to maintain safe and appropriate living environments.
  • Reports maintenance needs and household supply or furniture replacement needs according to agency policy.
  • Assists residents in transitioning to more or less restrictive levels of care, ensuring continuity of services and funding.
  • Ensures resident rights, confidentiality, and access to services are consistently upheld.
  • Participates in agency orientation regarding program operations, policies, procedures, and goals.
  • Attends staff meetings, treatment team meetings, provider meetings, and required staff development or training sessions.
  • Performs other duties as assigned to support program operations and resident success.

Requirements

Qualifications

Education: Associate’s degree in human services or related field preferred.

Credentials: Valid NY State Driver License and access to a reliable vehicle

Experience: Minimum of two years’ experience in Human Services or related field

Relevant years of experience and education will be considered

  • Willingness to work a flexible schedule, based upon needs of the Agency.
  • Possess excellent verbal and written communication skills.
  • Ability to maintain absolute confidentiality regarding all records reviewed including consumer records, employee records and billing records.
  • Possession of a valid NYS Driver’s license and a driver's record considered acceptable by agency and insurance carrier.
  • Continuous use of a reliable, registered and insured vehicle.
  • Correctly follow procedures for mandated reporting and responding.
  • Background checks may be required per program regulations.
  • Ability to demonstrate behaviors and attitudes which support organizational mission, philosophy, and policies
  • Ability to work in a cooperative and helpful manner with all individuals
  • Ability to understand the concept of "mandated reporter" and is diligent in reporting situations to the proper authorities when children may be at risk for abuse and/or neglect.
  • Correctly follow procedures for mandated reporting and responding
  • Willingness and ability to foster agency and program wide cooperation and teamwork through use of positive/constructive communication techniques
  • Adheres to all applicable federal and state laws and regulations including, but not limited to, those governing confidentiality, privacy, program, billing and documentation standards. All duties must be performed in accordance with CCDR’s corporate compliance and ethics program
