WAGE RANGE: $24.50 – $27.50/hourly

JOB SUMMARY: Works directly with assigned clients to develop the client's service plan. Adheres to case documentation requirements and works directly with clients to schedule and support client assessments with the team. Tracks client progress and adjusts service plan as necessary. Serves as an advocate for the client. Provides education related to services and challenges to the community.
ESSENTIAL FUNCTIONS
- Complete calls to clients to evaluate current services and make updates to care plans when appropriate.
- Supports and advocates on behalf of clients in the procurement of services and benefits entitlement, assisting with required paperwork as necessary.
- Provides education and referrals about available community resources and services to a complex, often long-term group of assigned clients.
- Maintains complete case documentation and records, as required by Catholic Charities, the funders, or licensure/regulatory bodies.
- Coordinates with the Administrative Assistants, Case Managers, Social Workers, and Supervisors on all aspects of client care and seeks appropriate guidance and feedback from other team members.
- Utilizes behavior management, trauma-informed, person-centered, and solution-focused techniques to facilitate personal growth and minimize crisis.
Other duties as assigned.
QUALIFICATIONS/TECHNICAL EXPERTISE
- Associate degree in human services or related field preferred and one (1) year of experience working with a specific client group.
- In lieu of degree, a minimum of two (2) years of experience working with a specific client group is required.
- Additional training, and/or education may be required per program need.
- Ability to work with diverse and/or low-income populations, including those who may be experiencing homelessness, unstable housing, and/or mental illness required.
