Case Manager 035 - New Brunswick NJ

Salary: $45,000 per year

Key Responsibilities

• Provide case management services to individuals residing in the Ozanam Inn Shelter and Permanent Supported Housing
• The job includes screening, assessment, service planning, linkage, crisis intervention, and de-escalation, managing relationships with a broad range of community providers, and life skills teaching
• Transporting individuals to housing opportunities, assisting client in applying for housing, and building connections with other agencies
• Utilization of HMIS (WellSky) case management software and MyEvolv.
• Requires on-call coverage and evening hours.
• Bi-lingual (Spanish) encouraged to apply.
• Ability to work independently.
  
  

Requirements:

• Bachelor’s degree (B.A) from an accredited college or university, preferably in Social Work or a similar Human Service field is preferred
• At least two (2) years relevant work experience and demonstrated ability to provide effective services to clients required
• Experience with conflict resolution and mediation.
• Must have valid driver’s license and clean driving record.
