General Responsibilities

• Case Manager conducts determination of eligibility assessments (DOEs) for older adults seeking waiver services while being enrolled with a Managed Care Organization.
• Active intervention, advocacy and social work to access necessary services from various programs and complete any necessary referral forms.
• Complete proper termination of cases.
• Maintain confidential case records and meet documentation standards.
• Accurate completion of all required reporting and documentation.
• As a Case Manager, maintain cooperative relationships with service providers and community organizations, including regular contact with Managed Care Organizations.
• Submit accurate files to billing in a timely manner.
• Attend training, staff, and team meetings, and act as an Agency ambassador in the community.
• Other duties as assigned within the guidelines of this position.

Requirements:

• Bachelor's degree in social work, psychology, or related field from a regionally accredited institution. In some cases, a waiver may be granted by the Illinois Department on Aging if employee has significant case management or significant experience in aging services.
• Minimum of three months of experience in a related field.
• Preferred experience with program-specific populations.
• Fundamental communication and problem-solving skills.
• Ability to work independently and respond appropriately in crises.
• Relate well to individuals with disabilities and older adults, respecting their privacy and dignity.
• Effective and constructive interaction with diverse cultures and backgrounds.
• Proficiency in Microsoft Office applications (Outlook, Word, Teams).
• Strong verbal and written communication skills.
• Adhere to the Agency’s Code of Ethics and conduct all responsibilities in accordance with the standards outlined in the Employee Handbook.
• Valid driver's license, reliable transportation, and proof of liability insurance.
