Position Description:
The Case Manager for Refugee Health & Education Outreach assists refugees with healthcare and health-related education under the Refugee Health Education & Outreach program. He/she plays a vital role in identifying resources that benefit the health & wellness of our clients. This position ensures clients receive an orientation about navigating the US Health Care System and help with understanding treatments/referrals.
Position Responsibilities:
While performing the duties of this job, the employee may be regularly required to stand, sit, talk, hear, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard. Specific vision requirements due to computer work and driving a motor vehicle. Light to moderate lifting up to 25 lbs. may be required.

Position Description:
The Case Manager for Refugee Health & Education Outreach assists refugees with healthcare and health-related education under the Refugee Health Education & Outreach program. He/she plays a vital role in identifying resources that benefit the health & wellness of our clients. This position ensures clients receive an orientation about navigating the US Health Care System and help with understanding treatments/referrals.
Position Responsibilities:
- Conduct a comprehensive health orientation for newcomers to assist clients with navigating the U.S. Healthcare system.
- Following refugee health screenings, assist with accessing care and treatment as needed.
- Provide support & assistance to agency staff with client physical and mental health issues.
- Support clients with any health barriers impacting self-sufficiency and integration.
- Facilitate delivery of mental health services by coordinating with the Community Services Boards and other providers.
- Coordinate regular support and wellness groups for clients.
- Deliver training for providers and clients.
- Time sensitive & mandatory data entry requirements: Virginia Database, VA Newcomers Information System
- Actively participate in meetings and trainings with stakeholders.
- Commitment to the mission & goals of Catholic Charities.
- Effective and adaptive oral & written communication skills in English. Ability to adapt communication style to meet the needs of a culturally diverse audience.
- Bilingual in English and another language spoken by clients required: Dari, Pashtu, Arabic, Amharic, & Spanish
- Flexible to work irregular hours.
- Valid US drivers license, 3 years of licensed driving and a good driving record. Willing to drive clients.
- Fluency in data entry and Microsoft Office suite.
- Knowledgeable about trauma informed care
- Associates degree with 3-5 years’ experience required or Bachelor’s degree in social services field preferred.
- Previous direct work experience in a culturally diverse environment. Preferably refugee resettlement
