For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness.
Through the Benson Ridge and the Southwest Queens Senior Case Management Programs, case management and the homecare services are offered to the homebound elderly. The services include offering care management plans, assessments for the homecare, connections with the Home Delivered Meals and Friendly Visiting. The goal of the Case Management Program is to provide vital support services to keep frail seniors living safely in their homes and communities.
STATEMENT OF THE JOB
Under the direction of the Project Director the Case Aide renders professional services as a member of a team to frail, homebound older adult clients. The Case Aide works in close collaboration with supervisor(s), Program Director, and the case management staff to deliver holistic services to said clients. Assess needs and provide information, referral, and case assistance to seniors.

Assess needs and provide information, referral, and case assistance to seniors.
- Collaborates with case management staff with client-related service tasks including making monitoring calls and home visits.
- Conducts intakes of new clients.
- Advocates and intervenes with other agencies on behalf of clients.
- Inputs data into program and funding source databases.
- Participates in outreach.
- Maintains client case records in audit readiness at all times.
- Attends staff meetings, trainings, and supervisory sessions, as required.
- High School Diploma/GED and four years of experience in a social service setting or in providing direct services to individuals or families.
- Able to conduct home/field visits and travel throughout the five boroughs as needed.
- Ability to lift up to 10 lbs.
- Bilingual preferred.
